Summary
"As the current Case Management Society of America (CMSA) National President, it is my distinct honor to introduce this update to the seminal CMSA's Integrated Case Management. CMSA has been a pioneer in the pursuit of true patient-centered care using an integrated approach; beginning with the development of content and curriculum in 2008. Fifteen years later, the ICM has become even more relevant to the practice of professional case management and the healthcare system overall. The goal of Integrated Case Management (ICM) is to provide a seamless, wholistic care experience for our patients/clients/members by integrating physical and mental health case management principles and assessment tools. Through this approach, the foundational principles of case management (right care, right setting, right time, reduction of duplication of services, resolution of care gaps, and patient advocacy) are optimized"-- Provided by publisher.
